Diuretics, also known as water pills, are medications that help the body get rid of excess salt and water.

Diuretics treat heart failure, high blood pressure, kidney disease, and swollen tissues. They help these conditions by reducing the extra fluids that build up in your body due to weakened blood vessels or organs that aren’t working efficiently, such as the heart and the kidneys.

How long it takes diuretics to work depends on different factors, such as the type of diuretics and what symptom or condition they’re treating.

Diuretics usually get to work fairly quickly within your body — often within a couple of hours. However, it may be several weeks before you notice their effects on your symptoms. There are three types of diuretics:

Thiazide diuretics

Thiazide diuretics help lower the amount of water and sodium in the body by blocking the reabsorption of sodium in your kidneys. This promotes the production of urine and helps your body get rid of sodium.

This type of diuretic also widens your blood vessels, letting blood flow more easily. Doctors use thiazide diuretics to treat high blood pressure. One of the most commonly used thiazide diuretics is hydrochlorothiazide (Microzide), and doctors may also use thiazide-like diuretics like chlorthalidone (Thalizone), and indapamide (Natriix).

Thiazide diuretics like hydrochlorothiazide start to work within 2 hours and continue to work for up to 12 hours.

Loop diuretics

Loop diuretics increase the salt and water you get rid of in your urine. These medications work on the part of the kidney known as the loop of Henle to achieve this.

Loop diuretics may lead to hypokalemia (low potassium levels) because their functioning leads to more potassium being expelled in your urine.

Loop diuretics can help lower blood pressure, treat heart failure, and relieve edema, also known as water retention. The most commonly used loop diuretics include furosemide (Lasix), torsemide (Demadex), bumetanide (Bumex), and ethacrynic acid (Edecrin).

Loop diuretics generally take an average of 30 to 60 minutes to start working, but how long they stay in your body depends on the individual drug.

Potassium-sparing diuretics

Potassium-sparing diuretics also help get rid of fluid, increasing urine production, but they won’t get rid of potassium (something that other diuretics do).

This type of diuretic lowers blood pressure while maintaining your blood potassium level. If potassium levels drop too low, it can lead to irregular heartbeat, or arrhythmia.

The most common potassium-sparing diuretics include spironolactone (Aldactone), amiloride (Midamor), and eplerenone (Inspra).

It takes longer for potassium-sparing diuretics to work because they’re weaker than other types of diuretics. Spironolactone, for example, gets to work within about 4 hours.

Signs they’re working 

You’ll know your diuretics are working when you experience relief or a reduction in symptoms. While diuretics generally take a few hours to exert their effects on your body, it might be several weeks before you notice an effect on your symptoms.

The most common side effect of taking diuretics is more frequent urination, and this might be one of the first things you notice to let you know the drug is working. 

Other signs that the medications are working depend on the condition it’s treating. For example, if you’re taking diuretics for edema, you’ll likely experience reduced swelling of your limbs and ankles. If you have fluid buildup in your lungs, you may breathe more easily.

Adequate hydration can also support your treatment of conditions like hypertension. For example, dehydration can raise your blood pressure, so drinking water can help reduce spikes if you have hypertension.

However, if you have heart failure, doctors will likely recommend limiting your fluid intake, as this may counteract the effects of the diuretic.
