Fibromyalgia syndrome is a chronic medical condition that involves widespread musculoskeletal pain, often accompanied by other physical, cognitive, and emotional challenges. 

Common symptoms of fibromyalgia include:

  • fatigue (low energy)
  • sleep disturbances
  • cognitive difficulties (fibro fog)
  • physical pain

Antidepressants for fibromyalgia pain

Antidepressant medications may help you manage fibromyalgia pain by affecting neurotransmitters in the brain that regulate pain perception. Improving your sleep quality can also lower pain and discomfort over time. 

Antidepressants are manufactured and approved to treat symptoms of depression. That is their label use.

However, healthcare professionals may sometimes prescribe antidepressants off label. This means for purposes other than indicated on the label.

Off-label uses of antidepressant prescriptions include anxiety, sleep disturbances, and chronic pain.

The only antidepressant medication approved by the Food and Drug Administration (FDA) to treat both depression and fibromyalgia is duloxetine (Cymbalta).

Duloxetine is a serotonin-norepinephrine reuptake inhibitor (SNRI) that affects how your brain reabsorbs serotonin and norepinephrine. These neurotransmitters are involved in functions such as heart rate, blood sugar levels, mood regulation, and pain sensitivity.

Other SNRI medications that your healthcare professional may recommend for fibromyalgia symptoms include:

  • milnacipran (Savella): This is the only FDA-approved SNRI to specifically treat fibromyalgia, but not commonly taken to treat depression.
  • venlafaxine (Effexor XR): FDA-approved for depression, but prescribed off-label for anxiety, chronic pain, migraine, diabetic neuropathy, and others.

Other medications prescribed to treat fibromyalgia symptoms include tricyclic antidepressants (TCAs). These also affect levels of serotonin and norepinephrine in the brain, although the acting mechanism is different from SNRIs.

TCAs may alleviate pain perception and promote better sleep quality in people with fibromyalgia.

The most common TCA prescribed to manage fibromyalgia symptoms is amitriptyline (Elavil).

Relief from fibromyalgia pain may become noticeable after a few weeks of consistently taking antidepressants. However, effects may vary from person to person.

Antidepressants for fibromyalgia psychological symptoms

Antidepressants may alleviate physical pain and effectively manage the psychological symptoms of depression and anxiety, which often accompany the condition. 

Fibromyalgia and depression often coexist, with chronic pain and disrupted sleep patterns contributing to these symptoms. 

Antidepressants — including selective serotonin reuptake inhibitors (SSRIs) and SNRIs — boost the levels of important neurotransmitters involved in mood regulation.

Anticonvulsants

Anticonvulsants are often approved to treat epilepsy, but they may also help regulate pain signals in people with fibromyalgia.

Anticonvulsants work by modulating how your nerves perceive stimuli, inhibiting the overactivity of pain-transmitting nerves.

Healthcare professionals commonly prescribe sleep medication for people with fibromyalgia. The most common one is trazodone (Molipaxin). 

Trazodone is actually an SSRI antidepressant commonly taken for sleep disturbances.

  • drowsiness
  • potential for dependence (needing a higher dose to get the same result)
